We respect and are committed to protecting the privacy of every patient. This privacy policy sets out how we protect and use information that we collect, or that you provide to us at your first and any subsequent appointments.
To provide patients with a high standard of care and attention, we need to hold their personal data which can include:
- past and current medical and musculoskeletal conditions; personal details such as fore- and surnames, age, address, telephone number, email address and general medical practitioner
- information about treatment that we have provided
- any correspondence relating to you with other healthcare professionals
We do not sell, disclose, share, or forward your personal data to any third parties, other than, with your permission, to relevant professional parties, such as your general medical practitioner, radiology, pathology departments, or other consultants to whom we may refer you. Such disclosure will take place on a ‘need-to-know’ basis (only that information that the recipient needs to know will be disclosed).
We adopt appropriate data collection, storage and processing practices and security measures to protect against unauthorised access, alteration, disclosure of your personal information and any data stored.
You have the right, at any time, to:
- access the information we hold about you in accordance with the Data Protection Act 1998
- ask for copy of some, or all of the personal information we hold on you
- ask for your information to be corrected, or updated
- ask to remove/permanently delete your information from our records
